What was the source from? MKV is just a container, and it is treated the same as any other container by ffmpeg (which is what placeshifter uses).

MakeMKV ripped from the original DVD - not a copy.

Can anybody name some other playback utility that uses ffmpeg? Maybe testing with that would tell us something.

From a DVD, it shouldn't be a problem. I was more concerned about a BluRay of HD-DVD rip. The basic MPEG-2 in the DVD shouldn't have any problem in placeshifter.
