I got a reply to my bug report that the USB-UIRT plugin is not written by then, so I need to contact that USB-UIRT support for help, which I've done.

Reading back through the thread, it looks like your issue is not the same one I'm referencing if you're only using the USB-UIRT as a blaster and not as a receiver. The bug appears to be when trying to use it as both at the same time while in service mode on Vista.

I don't have Vista - but I have had similar head banging problems with the USB-UIRT on occasion.

You said this was a new install? Did you recreate your .IR files also?

When using one the channels off the back of the USB-UIRT the codes for the cable box must be prefixed with the ZONE. Z1 & Z2 are the outputs on the back. Z3 is the front.

The NAME of the USB file must be contained in the IR file itself ...I can't tell you how many times I've forgotten that little trick.
This is the first line from one of my .ir files:
QIP62-1 0 0 4 600 800
The name of the file is: QIP62-1.ir
