I use DVDFab to rip the main movie and then I use a program called vobmerge to combine the entire movie in to a single file. I rename that file to a .vob but it writes it as a mpg. It works very well. I use DVDFab to rip blurays now. It is very convenient when it makes a single m2ts file or whatever it is called but I use a different program called tsmuxer to combine the files in to one main file.
I hope that helps. I think it works very well. I don't have chapters but that doesn't really matter with sage saving the place where I left off. Also I can fast forward pretty quickly so chapters just aren't that important!

Now that v7.0.12 supports MKV chapters, MakeMKV is that much better of a choice.. ;-)
