recording quality not always being enforced

I have changed my default recording quality to Max Mpeg-2 last week.
For the most part everything has been recording fine. However a couple of recordings have recorded as 4Mbit/s instead of 12. I checked the video details and it does not state the recording quality like it usually does. I checked the Favorite settings for the recorded shows and they are both set to default for recording quality and so should use Max Mpeg-2. Any ideas as to what is going on?

What are the tuners? Maybe one or more of your tuners can't record at that rate.

2 of them are USB2's and the other 2 are on a pvr-500. The first time it happened it was on one of the tuners from the 500 so I checked to make sure that one of the tuners on the dual 500 was not the culprit but the last time it happened it was from one of the USB tuners.
But all of them are Hauppauge and capable of 12Mbit/s

There was a problem getting PVR-USB2 tuners to set their recording rate correctly several months ago. But, as long as you are running the latest v5.0 version, that should be fixed. I'm not sure what else would cause it, but a log might give you some info.
