Tuner no signal - fixed (kind of)
Ok, this is kind of interesting, and maybe some good anecdotal information for someone else who may have a similar issue.
I use the PVR-500 and PVR-250.
I ran into these problems when I rebooted my Ubuntu box and the tuners stopped working. In trying to fix that, I screwed up the entire machine, so I decided to try the official Gentoo release.
Gentoo and Sage installed great, but the tuners still didn't work! I saw that the ivtv drivers that Sage installed (v0.7.3)had been pulled from the ivtv site and the recommendation was to upgrade the kernel to 2.6.18+ and use the 0.10.1 drivers. So I did. That's where I ran into the issue where I couldn't get the ivtv drivers to load.
So, last night, I dusted of an old PC and redid the Gentoo install on that box. I only moved the PVR500 to it, just to test it out. I left the 2 400GB drives in the old, brokeback PC. When I went to bed, the tuners were working.
This morning, I set to work on moving over the media drives and setting up the new Sage system. When I booted up, the tuners no longer were working! Very frustrated, I tried a shot in the dark and removed the PVR250 from my system.
Everything works GREAT!
I have not tried using the PVR250 by itself. I'll try that some other time.
I also don't know if there is a problem with the PVR250 or if the installed ivtv drivers have IRQ conflict issues, which is why they recommend using v0.10.1 with a newer kernel.

Reason: after 48-hours of dependancy and version hell, I misreported up the ivtv version that Sage installed
