Suggested changes to Bug Submission Form

1. Ability to attach screenshots
2. Give an additional field to specify version upgraded from
3. Automatic email confirmation. Yes, I know we will receive no communication from Frey on bug reports, but an automatic email would be useful so I can track what I've reported and when.

More ideas guys?
