What is the red "R" over white?
I have noticed a new icon show up in the last week or so. It's a white square with a red cursive "R" inside.

What is this?

Not sure since it could be dependant upon UI mods or other stuff.

I would most likely guess it is an Error Message notification. Check your System Messages.

Or can you post a screenshot if that isn't it?

That sounds like you have the Recently watched plugin Installed.
