Refresh Library Not Working

I posted this same question in the Metadata Importer thread, but I don't think this is the problem, so I'm posting here also.

I rebuilt my Sage Server a couple of weeks ago, and had no problems until a few days ago when my library won't truly refresh anymore...

I haven't had any new media to verify that NOTHING is working with the refresh, but if I have deleted a file through the interface (or at the windows level), or moved a file to another folder, the SageMC interface still shows the OLD location/file, and even shows those that are no longer present.

Has anyone else seen this problem? I have not had this issue in the year that I've been using Sage, and I can't say that it's not related to the BMT plugin, but again I don't think it is...

Is there a database file somewhere that perhaps isn't getting updated properly, or a log file that is unable to grow? I have TONS of space (many 10s of GBs) on the server, and the drives are all physically inside the machine.
