How can I feed OTA and Cable to my HVR-2250

The HVR-2250 has just 1 RF input and then dual hybrid tuners that can handle both analog and digital (ATSC & QCAM). Is there a way to run both OTA and cable to the 1 RF input using a signal combiner or other solution? Or do I need to get a second card for ATSC?

You'll need a second card. They use the same freq. just different formats.

I'm looking at getting this card soon, too. So can you run one coax from the wall outlet into it and then one feed from an OTA into it and capture two signals from each? I think I made sense, but it's been a long day.

No. As stated, only one coax input. So either you can capture 2 QAM streams or 2 OTA streams, you can NOT do 1 of each.
